Key Responsibilities
Coordinate and oversee the delivery of safe, high-quality, patient-centered home health services.
Provide clinical guidance and support to field clinicians, addressing patient care questions and helping resolve clinical concerns.
Review, coordinate, and monitor individualized patient care plans, ensuring plans of care are current, appropriate, and updated as needed.
Triage patient and caregiver calls, assess clinical concerns, and appropriately prioritize and escalate issues requiring additional intervention.
Review, clarify, process, and accurately enter physician and provider orders into the EMR in a timely manner.
Communicate effectively with physicians, providers, clinicians, patients, caregivers, and interdisciplinary team members to coordinate care and support continuity of services.
Monitor clinical documentation for accuracy, completeness, and timeliness, including conducting chart audits and following up on identified deficiencies.
Ensure compliance with applicable home health regulations, clinical standards, Company policies, and procedures.
Assist with the onboarding, orientation, training, and ongoing support of new clinical staff.
Coordinate and monitor medical supplies and equipment, ensuring appropriate availability, organization, and compliance with applicable OSHA and safety standards.
Provide administrative and clinical support to promote efficient daily office operations and effective coordination between the office and field staff.
Identify operational or clinical issues that may affect patient care, staffing, or workflow and communicate concerns to appropriate leadership.
Qualifications
Active California RN License (Required)
Home Health experience (Required)
1–2 years of supervisory experience (Preferred)
BLS Certification (Preferred)
Excellent organizational, communication, and interpersonal skills
Strong computer proficiency (Apple products & EMR systems)
Ability to multitask and manage high call volumes with professionalism
Education
Associate Degree in Nursing (Required; BSN Preferred)
Work Setting
One on-site location in Walnut Creek, CA
